Completion of an exam does not guarantee certification, approval, employment clearance, immigration approval, or acceptance by any agency, employer, or third party. Final decisions may depend on medical findings, legal requirements, agency rules, employer policies, and documentation review. 

Patients are responsible for bringing required forms, identification, vaccination records, medication lists, and any other documents needed for their visit. 

Payments and Fees 

Patients are responsible for all fees, copays, deductibles, self-pay charges, and balances not covered by insurance or third parties.
